High-Impact Community Investments by Health Plans

Health plans are increasingly investing in programs to address their beneficiaries’ social determinants of health (SDOH) needs, such as leveraging community health workers (CHWs). To this end, ASTHO—in partnership with CVS Health®—executed a research plan that included a literature scan and focus group interviews with health systems, state health departments, and organizations developing and deploying innovative CHW models and supporting the CHW workforce. This paper explores the results of that research and describes how an insurer-sponsored CHW program can address health plan members’ SDOH needs.
