Health Opportunity Index: Using Data to Measure and Address Social Determinants of Health

Join ASTHO and state health leaders in learning more about a tool known as a Health Opportunity Index (HOI). Health departments in Ohio and Virginia have used this index to understand how communities are affected by the social determinants of health. HOIs are composite measures of these factors that show areas and populations that are most vulnerable to adverse health outcomes. This webinar will allow states to learn from the experiences of others who have been using the tool, focusing on specific ways HOI data can be used and sharing tips for those interested in replicating the steps taken.

Social Determinants of Health: Financing Interventions and Strategies for States

There is increasing evidence that points to the value of interventions that address the social determinants of health. Recognizing the impact these interventions have on achieving better health outcomes, healthcare providers and state health agencies are exploring sustainable financing options. During this webinar, presenters will describe how public and private insurers can finance interventions that incorporate social determinants of health and other social services. Presenters will also share examples of approaches that are currently in practice and what other states need to know before implementing them.
