This is the 2015 report from the Oregon Health Authority presenting available data on evidence-based SMP programs in Oregon as well as current reach and capacity.

Montana Telehealth Diabetes Prevention Program
This short ASTHO publication gives an overview of the reintegration of public health and primary care in Montana. The Montana Department of Public Health and Human Services and a community hospital partnered to offer a diabetes prevention program at the hospital and via telehealth.

The Guide to Community Preventive Services: Interventions for Diabetes Management and Control
Recommendations and findings regarding eight interventions to manage diabetes and improve glycemic control are provided in this fact sheet from ASTHO. They come from the Guide to Community Preventive Services. With oversight from the Task Force on Community Preventive Services Task Force, a team of experts led or supported by Community Guide scientists conducted a systematic review to evaluate scientific evidence and provide recommendations to policymakers on these interventions.

ASTHO Case Study: Healthy Hearts for Marylanders
This case study describes the collaboration in Maryland that provided technical assistance to Federally Qualified Health Centers across the state of Maryland. This tecnichal assistance was intended to achieve improvements in outcomes related to the ABC's of heart health among patients diagnosed with diabetes.

The Patient Pharmacists Partnership in Maryland is intended to improve hypertension and diabetes prevention and control through medication therapy management. This case study gives an overview of the initiative, identifies key partners, and outlines next steps.   

USPSTF Recommendations for Abnormal Blood Glucose and Type 2 Diabetes Screening
This report from the U.S. Preventative Services Task Force provides recommendations for abnormal blood glucose screening in adults. It also outlines the rationale and clinical considerations of the decision making process.

America's Diabetes Challenge
America’s Diabetes Challenge is a program from Merck, the American Diabetes Association, and iHeart Media that urges people with type 2 diabetes to pledge to work with their doctor to set and reach their A1C goals (average blood glucose over the past 2-3 months). The program also encourages people to learn if they are at risk of low blood glucose, known as hypoglycemia, and how to help reduce that risk.    

Rural Diabetes Prevention and Management Toolkit
This toolkit provides resources and best practices to identify, implement, and sustain a program to prevent and/or manage diabetes in rural areas.

Small Steps. Big Rewards. Your GAME PLAN to Prevent Type 2 Diabetes
This website is aimed at patients who are at high risk for diabetes. It provides information on how to lose five to seven percent of weight, the suggested amount for people at high risk for diabetes. Basic strategies are getting at least 30 minutes of moderate-intensity physical activity and reducing the number of calories eaten per day.  

Simple Steps to Preventing Diabetes
This resource from the Harvard School of Public Health is aimed at patients working to decrease their chances of developing type 2 diabetes. It provides a bottom line for preventing type 2 diabetes: stay lean and stay active.

National Standards for Diabetes Self-Management Education and Support
These standards for diabetes education and support are reviewed and revised every five years by key stakeholders and experts within the diabetes education community. Ten standards in total are outlined.
