Population Health Forum

Population Health Forum

The Population Health Forum (formerly known as the Integration Forum and the ASTHO-Supported Primary Care and Public Health Collaborative) served as a partnership of organizations seeking to inform, align, and support the implementation of integrated efforts that improve population health and lower healthcare costs. The Forum served as a platform for primary care, public health, and others to share ideas, learn about each other’s successes and challenges, and create closer connections.

Committee and All Partners Meeting Minutes

Forum partners met via four committees (Successes/Measures, Sustainability, Workforce, and Communications) and through calls with All Partners to share current innovations in the field, examine successful models of integration, and learn about new tools and resources to support integration. Review all meeting minutes from each of these calls.
