The Help End Addiction for Life Initiative

ASTHO/CDC Heart Disease and Stroke Prevention Learning Collaborative: Lessons Learned from the Payers Cohort

Through the ASTHO/CDC Heart Disease and Stroke Prevention Learning Collaborative, a six-state cohort (Arkansas, Florida, Nevada, Utah, Virginia, and Wisconsin), i.e., the "payers cohort," collaborated with public health departments and private payers to improve population-level blood pressure control through innovative, data-driven healthcare delivery and payment models.
