Health Systems Transformation

ASTHO’s work in health systems transformation promotes cross-cutting collaboration and integration in a variety of areas across the changing world of public health. ASTHO’s work supports the Triple Aim of improving the patient experience of care, improving the health of populations, and reducing the per capita cost of health care.

All efforts related to health transformation focus on promoting access. ASTHO's Access Policy Committee provides thought leadership and policy development on issues related to access to public health and health services, particularly for vulnerable and at-risk populations in order to achieve optimal health for all. The committee supports strengthening clinical and public health partnerships across community systems by focusing on maternal and child health, health equity and the social determinants of health, the public health infrastructure, the health workforce, and behavioral health. The Committee recently approved ASTHO’s white paper outlining its vision for a transformed health system in the 21stcentury

Community Health Workers

Cross-Sector Partnerships to Address Social Determinants of Health

ASTHO is working to help states, with the leadership of their state health agencies, to address the social determinants that affect our health status on a population level as part of our daily lives. Medicaid and Public Health Partnerships

ASTHO supports state health agencies by providing information and technical assistance to facilitate partnerships with Medicaid. These activities encompass a range of payment and delivery reform initiatives such as the State Innovation Models (SIM) Initiative and promoting collaborative opportunities between Medicaid and public health that will achieve the Triple Aim of better care, reducing costs, and improving population health outcomes. Primary Care and Public Health

ASTHO's Primary Care project examines key areas of intersection between public health and primary care, and supports the 2013 President's Challenge of the reintegration of public health and healthcare. ASTHO also addresses training and technical assistance for state primary care offices, and provides information to state health officials on larger access to care and primary care issues.
